What is a copayment and how does it work in health insurance?
Everything worth knowing about copayments: what they are, what they cost, how they're applied and when this type of policy suits you.
What exactly is a copayment?
A copayment is a fixed amount you pay directly each time you use a medical service covered by your policy. It's literally a "co-payment": you pay part and the insurer pays the rest.
A simple example: If your plan has a €5 copayment for specialist appointments and you see a dermatologist, you pay €5 for that visit. The rest of the cost (which might be €80-150) is covered by the insurer.
The copayment is separate from the monthly premium you already pay. It's an additional cost that only arises when you actually use a service.
How it works in practice
A typical scenario
- You have a policy with copayments and pay a premium of €45/month.
- You need to see an orthopaedic surgeon. You book through your insurer.
- You attend the appointment as normal.
- On the way out (or at month end, depending on the plan) you're charged the €5 copayment.
- If the surgeon orders an MRI, you're charged another copayment of €15 when you have it.
- Your total cost that month: €45 (premium) + €5 (appointment) + €15 (MRI) = €65.
When is the copayment charged?
It depends on the insurer and the plan:
- On the spot: You pay when you receive the service (most common for appointments).
- At month end: All that month's copayments are added up and billed with the premium.
- Direct debit: Charged to your bank account automatically.
Types of copayment and what they cost
Copayments vary by type of service. These are the usual ranges in Spain:

Why do copayments exist?
Copayments serve two main purposes:
1. Bringing the monthly premium down
By taking on part of the cost each time you use a service, the insurer can offer you a lower monthly premium. It's a trade: you pay less each month, but something each time you see a doctor.
2. Encouraging sensible use
Copayments discourage unnecessary use of medical services. When there's a small cost attached to each appointment, people tend to go when they genuinely need to.
When does a copayment plan suit you?
Policies with copayments are the better option if:
- You're young and healthy: You see a doctor 2-4 times a year. Copayments will be minimal.
- Your budget is limited: The priority is the lowest possible premium.
- You use it as a top-up: You have public healthcare and only use private cover occasionally.
- You're insuring just yourself: No children or dependants multiplying the visits.
When a copayment plan does NOT suit you
Copayments aren't ideal if:
- You see a doctor often: They add up quickly.
- You have young children: Children see the paediatrician and A&E a lot.
- You have a chronic condition: Regular monitoring generates a lot of copayments.
- You want complete peace of mind: You don't want to think about cost when you need care.
Is there a cap on copayments?
Some plans set an annual limit (for example, €300 a year). Once you hit that ceiling, you're not charged for the rest of the year. Not all plans offer this protection, though, so it's worth checking before you commit.
A real comparison
Someone aged 35 who sees a doctor 8 times a year (4 appointments + 2 tests + 1 A&E visit + 1 physiotherapy session):

In this case the copayment plan is €360 cheaper a year. But if that same person had 15 visits a year, copayments would rise to around €120, narrowing the gap to €300.
