How much does private health insurance cost in Spain?
A price guide updated for 2026. The real ranges by age, plan type and situation. No surprises.
What drives the price
The cost of private health insurance in Spain isn't fixed. Several factors combine to set your personal monthly premium.
1. The age of the person insured
This has the biggest impact. The older you are, the higher the premium, because the likelihood of needing medical care and the use of services both rise over time. The difference between someone aged 25 and someone aged 65 can be three to five times the premium.
2. Level of cover
A basic plan with copayments will be significantly cheaper than a premium plan with no copayments, reimbursement and dental cover.
3. With or without copayments
Plans with copayments have premiums 20% to 40% lower than the equivalent without.
4. Where you live
Premiums vary by province. Big cities (Madrid, Barcelona) tend to be slightly more expensive, though they also have more choice and more competition.
5. How many people are insured
Family plans offer discounts for each additional member, and many include children free or at reduced rates.

How to bring the cost down
- Always compare: Differences between similar plans can be 30-40%.
- Choose copayments if you're young and healthy: The premium will be far lower.
- Start young: Premiums are lower and, with a stable-premium plan, they stay contained.
- Don't pay for cover you don't need: If you don't need dental or travel cover, don't buy it.
- Use family discounts: Insuring everyone together costs less.
- If you're self-employed, claim the tax relief: You can deduct up to €500 a year.
