The Spanish health system: public and private

Public healthcare

The National Health System (SNS) provides universal cover to everyone resident in Spain. Its clinical quality is internationally recognised, but it has limitations in the form of waiting lists which, for certain specialities and diagnostic tests, can stretch to weeks or months.

Private healthcare

Spain's private health sector has an extensive network of hospitals, clinics and professionals. More than 11 million people hold private health insurance, roughly 24% of the population.

Types of private health insurance

By level of cover

Basic: The most affordable option. Includes primary care, specialists and basic diagnostics with copayments. Ideal for younger, healthy people on a tight budget. From €30/month.

Essential: Mid-range cover with moderate copayments. Includes hospital stays, surgery and a wide specialist network. Its strength is stable premiums over the long run. From €50/month.

Complete: Comprehensive cover with no copayments. Access to the whole private hospital network and to specialists with no extra charge per appointment. From €80/month.

Premium: The top of the market. No copayments, reimbursement of medical costs, full dental cover and international healthcare assistance. From €120/month.

By who it's for

Family cover: Plans with paediatric and maternity cover plus group discounts.

Cover for older people: Adapted to the needs of people over 60, with home care and specific benefits.

Cover for foreign residents: No waiting periods, repatriation cover included and valid for residence permits.

The key terms to know

Copayment

A fixed amount you pay each time you use a service (for example, €5 for a specialist appointment). Plans with copayments have lower monthly premiums.

Premium

The monthly amount you pay for your cover, whether you use it or not.

Cuadro médico

The list of hospitals, clinics and professionals you can use with your policy. Before you commit, check there are good centres near where you live.

Waiting period

The time between taking out cover and being able to use certain benefits. For example: hospital stays (3-6 months), surgery (6-8 months), maternity (8-10 months). Primary care is usually available from day one.

Exclusions

Services or situations the policy doesn't cover. The most common: cosmetic surgery, experimental treatments, high-risk sports.

Reimbursement

A system that lets you see professionals outside the network and claim back a percentage of the cost (usually 80% to 100%). Only available on premium plans.
